Dentists who have chosen to be a Preferred Network
Provider will accept the reduced fees listed on the
Network Fee
Schedule, in your area, for Junior and Senior Members. |

2. Associate Network
Providers (ANP)
|
| Dentists who have chosen to be Associate
Network Providers offer network members a discount off their
regular rates as follows: |
Junior Members
(age 61 and under) |
20% discount on *Minor procedures and
30% discount on **Major procedures. |
Senior Members
(age 62 and over) |
25% discount on Minor procedures and
35% discount on Major procedures. |

Associated Providers
who do not offer the above discounts, have the level of discount percentage
they are willing to extend to our members stated below their name on the
Providers List.
In this content, *Minor Procedures are procedures that cost
less then $1,000 and **Major Procedures are procedures that cost more then
$1,000 at the treating provider's usual charges for the same services.
A single treatment that requires several
Minor procedures that total more then $1,000, will be considered and billed at the major procedure discount
level.

Provider's fees may vary, always inquire before starting treatment.
