** If only one spouse is 62 or over, you will qualify for
the Senior Couple Membership Fee.
*** Family constitutes parent's and any children living at
home. No age limit on children.
*** Additional members cost is total cost regardless of
number of additional members.
Please be advised that the Fee Schedules
are based on your age. If you are under age 62 and qualify for the
Senior Couple or Senior Family Membership Rate because your spouse or
parents are over age 62, you will be responsible for paying for the dental
procedure at the GADN Basic rate until you reach age 62. |
