Eligibility and Membership Plans Available
Membership in the Golden Age Dental Network is open to anyone
without any regard to age
(providing the
principle subscriber is eighteen years of age or older). Membership Plans are
available for singles, couples and families. Basic Memberships are
for members under 62 and Senior Memberships are for people 62
and older. For a complete list of membership plans available
and rates, please refer to the Membership
Plans section.
Effective Date of Membership
Your membership is effective the date your application is received. We will notify you immediately either by phone or by email.
Be sure to provide your home phone number and your email address, if you
have one, on your application form. We will then mail you a membership
packet containing a receipt and a membership card for all plan participants.
Length of Membership
The minimum length of a membership in the Golden Age Dental Network is
one year and renews automatically unless cancelled in writing by the
principle member.
Signing up
Before signing up for the GADN, we suggest you read our
Disclaimer and
make sure there is a
GADN Participating
Provider in your area. You should also check the Membership
Plans section to determined rates for the type of membership desired.
You may then enroll online with a check or by mail.
Cancellation of Membership
You may cancel your membership in writing, when annual renewal is due. We
reserve the right to cancel any member for lack of payment sixty days
after payment is due.
Signing up after Membership Cancellation
In order to prevent members from canceling and signing up again when Dental
Care is needed, all members who have canceled in any form and would
like to sign up again, will be required to pay all membership fees from the
date of cancellation or two years worth of membership fees, which ever is
less.
